He said the novel app-based behavioral therapy may appeal to the many men who avoid medical help because of stigma.

While the app does not cure the condition, added Groeben, it helped men manage it more effectively. “The most important thing is to improve the way men think about it and handle their sexuality and, with this, improve their sexual quality of life and that of their partners.”

The findings were presented as a poster at the European Association of Urology (EAU) 2026 Annual Meeting.

CLIMACS Trial of the Melonga App

Premature ejaculation is one of the most common sexual dysfunctions in men and is typically defined as ejaculation sooner than desired during sex (within about a minute of penetration) and often accompanied by distress or relationship difficulties. Despite its prevalence, only about 9% of men seek medical help, according to the International Society for Sexual Medicine’s guidelines for the diagnosis and treatment of premature ejaculation.

The CLIMACS trial evaluated whether the Melonga app, a digital therapeutic tool developed by urologists and psychologists, could help men manage the condition through behavioral training delivered at home. The program teaches techniques commonly used in clinical therapy, including mindfulness, arousal-awareness exercises, cognitive-behavioral strategies, and physical control techniques such as the start-stop method.

Groeben explained that the app addresses the psychological and relationship aspects of sexual function. “One third of the content of the app is about talking to the partner and finding other ways to express sexuality beyond the penetrative act,” he said in an interview with Medscape Medical News.

The study enrolled 80 men without other underlying health conditions and randomly assigned them to either use the app or no intervention for 12 weeks. The primary endpoint was the change in the Premature Ejaculation Profile score over the study period. Participants also measured their intravaginal ejaculation latency time (IELT) using a stopwatch and completed validated questionnaires assessing symptoms and sexual quality of life (the Premature Ejaculation Diagnostic Tool and the Sexual Quality of Life score, respectively).

Clear Benefit With App Use — Ejaculation Latency Time Lengthens

Among the 66 men who completed full datasets, the results showed clear improvements in those using the app vs the control group. The Premature Ejaculation Profile score increased by 4.4 points in the intervention group vs 0.03 in the control group (P < .001).

The total IELT increased by an average of 64 seconds (from 61 to 125 seconds; P < .001), with a mean difference of 75.7 seconds in the intervention group and 0.5 seconds in the control group after 12 weeks (P = .001). The Sexual Quality of Life scores increased from 32.8 to 43.1 points (P < .001), with an increase of 13.4 points in the intervention group and -0.1 in the control group (P = .009).

Scores on the Premature Ejaculation Diagnostic Tool decreased substantially, indicating fewer symptoms; the mean score decreased from 16.4 to 11.9 points over 12 weeks (P < .001). Overall, 22% (15/64) of men who used the app no longer met the diagnostic criteria for premature ejaculation after 12 weeks, based on self-reported measures.

Potential Use in Combination With Medication or Psychotherapy

Current treatments for premature ejaculation often involve topical anesthetic creams or medications designed to delay ejaculation physiologically. However, Groeben said these therapies do not necessarily address psychological triggers such as anxiety or arousal control. “The app is not meant to replace pharmacotherapy or psychotherapy, but it can work in combination with these approaches,” he said, adding that future research should examine whether combined approaches lead to greater improvements.

Commenting on the findings, Giorgio Russo, MD, associate professor of urology at the University of Catania in Catania, Italy, and chair of the European Association of Urology’s Young Urologist Office, said the results suggest digital therapies could help bring reliable guidance to men navigating a confusing landscape of online advice.

“There’s much information and misinformation out there for men who find they have premature ejaculation,” Russo said in a statement. The app, he said, brings together effective advice from urologists and psychologists into a single trusted resource. He added that the results are notable because the intervention improved symptoms without medication. Larger studies should also examine the impact on partner satisfaction.

The CLIMACS investigators said the findings support expanding digital behavioral therapy as part of the treatment options for premature ejaculation. The Melonga app is currently available in several European countries — including Germany, Ireland, Austria, Luxembourg, Liechtenstein, and Belgium — and final peer-reviewed results from the study are expected later this year.

The CLIMACS study was funded by Prognoix Health BV.
