Median AD severity scores included 34 based on the SCOre Atopic Dermatitis (SCORAD) scale and 14 on the Patient Oriented Eczema Measure (POEM) scale. <\/p>\n<p>These patients completed surveys at baseline and again between 5 and 7 months after they began treatment for their AD recommended by an AD specialist. <\/p>\n<p>The survey included three international scales that explored initiating and maintaining relationships (the relationship domain), feelings about sex and sexual stimulation (the sexual desire domain), and initiation, enjoyment and frequency of sexual intercourse (the sexual activity domain), as well as items about the impact of AD on sexual activity. <\/p>\n<p>In addition to the 85% of patients overall reporting a sexual health disorder, as well as 57.4% reporting disorders in at least two domains, 60.6% reported a disorder in the relationship domain, 54.7% reported a disorder in sexual desire and 42% reported a disorder in sexual activity. <\/p>\n<p>Based on severity as defined by SCORAD, 56.5% of those with mild AD (&lt; 25 points), 96.4% of those with moderate AD (26-50 points) and 100% of those with severe AD (&gt; 51 points) reported a sexual health disorder. <\/p>\n<p>Men were more likely to report disorders in the relationship domain, including difficulties in maintaining relationships, and women were more likely to report disorders in sexual desire and activity, such as unpleasant and painful sexual activities and negative emotional impact, the researchers said. <\/p>\n<p>Risk factors for these disorders included POEM severity (OR = 4.67; 95% CI, 3.05-5.79), SCORAD severity (OR = 3.88; 95% CI, 2.68-4.73), longer disease duration (OR = 3.75; 1.88-4.91) and eczema on the face, hands or genital area (OR = 3.15; 95% CI, 2.88-5.19). <\/p>\n<p>Improvements in AD severity and disorder frequency followed pharmacotherapy, the researchers said. Patients with mild AD had the largest improvements in relationships and sexual desire. Those with moderate AD had the biggest improvements in sexual activity. <\/p>\n<p>Overall relative reductions in disorders included 60% for the mild group, 41% for the moderate group and 28% for the severe group. <\/p>\n<p>Specifically, relative reductions included 62.5% for relationships, 68.8% for sexual desire and 48.6% for the mild group; 11.3% for relationships, 59% for sexual desire and 53.6% for sexual activity for the moderate group; and 34.5% for relationships, 35% for sexual desire and 14.3% for sexual activity for the severe group. <\/p>\n<p>The researchers further noted a significant correlation between SCORAD score and quality of life based on these disorders at baseline (R = 0.814; <i>P<\/i> = .001) and after pharmacotherapy (R = 0.916; <i>P<\/i> &lt; .001). <\/p>\n<p>Based on these findings, the researchers said that even mild AD can have a negative impact on sexual health, but effective disease management can mitigate these risks, reduce the prevalence of these disorders and improve quality of life. <\/p>\n<p>Although emotional, cultural and social factors make discussions about sexual health challenging, the researchers said, clinicians still should assess their patients for these disorders, particularly patients with chronic diseases and other risk factors. <\/p>\n<p>For example, the researchers said that more than 98% of the patients in this study said that they had never been asked about their sexual health before this survey. But these findings can enable interventions that target these patients as well as holistic approaches for AD management, the researchers added. <\/p>\n<div class=\"article__content--footer\">\n<div class=\"perspective\">\n<h2 class=\"title\">Perspective<\/h2>\n<p>    Back to Top <i class=\"far fa-arrow-up\"\/> <\/p>\n<div class=\"perspective-body\">\n<div class=\"img-group\">\n            <img decoding=\"async\" class=\"perspective-img\" src=\"https:\/\/www.healio.com\/~\/media\/slack-news\/allergy_asthma\/mugs\/g\/ghozland_david_80x106.jpg?w=80\" alt=\"David Ghozland, MD\"\/>\n        <\/div>\n<p>      <default:p xmlns=\"http:\/\/www.w3.org\/1999\/xhtml\">The findings themselves aren\u2019t surprising. Intuitively, it makes sense that visible eczema and itching and obvious skin conditions would interrupt intimacy. But what did shock me was the sheer scale of the problem and how entirely ignored it is by the medical community. <\/default:p><default:p xmlns=\"http:\/\/www.w3.org\/1999\/xhtml\">The researchers found that 85% of all participants experienced sexual health disorders, but only five prior studies had examined this connection between AD and intimacy. It\u2019s also a sad reality that 98% of the patients in this study said this was the very first time a doctor had ever asked them about their sexual health. <\/default:p><default:p xmlns=\"http:\/\/www.w3.org\/1999\/xhtml\">I want to say this is not the real case, but I see this exact disconnect constantly at my Santa Monica practice. Women come in for labiaplasty and eventually confess how it has impacted their intimacy for years. They never brought it up to their previous doctors because no one asked about their sexual health. They assumed their medical providers only cared about treating diseases and not the toll they take behind closed doors. <\/default:p><default:p xmlns=\"http:\/\/www.w3.org\/1999\/xhtml\">The demographic gap in the respondents is also not surprising. I can\u2019t count how many of my patients come in for consultations and end up asking questions on behalf of their husbands.<\/default:p><default:p xmlns=\"http:\/\/www.w3.org\/1999\/xhtml\">From my experience, patients won\u2019t volunteer certain information face-to-face because of shame and cultural stigma, but they will check boxes on a form. My advice would be to use tools like the Female Sexual Function Index or Massachusetts General Hospital-Sexual Functioning Questionnaire SFQ in your intake forms before the appointment even starts. (The extra anonymity will help prior to seeing a doctor.) <\/default:p><default:p xmlns=\"http:\/\/www.w3.org\/1999\/xhtml\">During consultations, even if nine out of 10 patients aren\u2019t forthcoming, still, continue asking. And when you do discuss results, normalize it immediately by saying, \u201cI ask everyone this because sexual health directly affects treatment outcomes and quality of life.\u201d That single sentence removes the idea that you\u2019re singling them out or judging their condition.<\/default:p><default:p xmlns=\"http:\/\/www.w3.org\/1999\/xhtml\">The study tracked patients for only 4 to 7 months, which tells us nothing about whether these sexual health improvements (they claim) actually last. AD is a chronic relapsing condition, so I want to see longitudinal studies following patients for years to see if pharmaceutical control truly sustains intimacy gains or if symptoms return during flare cycles. <\/default:p><default:p xmlns=\"http:\/\/www.w3.org\/1999\/xhtml\">The study also focused almost exclusively on pharmacotherapy, although the researchers do acknowledge that anxiety and depression play major roles in sexual dysfunction. So, the next step can also be testing multidisciplinary protocols that combine dermatological treatment with psychological support from the start. <\/default:p><default:p xmlns=\"http:\/\/www.w3.org\/1999\/xhtml\">Finally, I need to mention something about this study\u2019s funding that caught my attention<default:i>. <\/default:i>Pfizer<default:i> <\/default:i>and Sanofi funded this study, and both manufacture AD medications. The authors state the funders didn\u2019t intervene in execution or writing, but that relationship still raises questions about whether the conclusions favor pharmaceutical interventions over other approaches. <\/default:p><default:p xmlns=\"http:\/\/www.w3.org\/1999\/xhtml\">They also claimed that treating AD reduces sexual health disorders, but personally I think this needs replication by independent researchers using non-industry protocols before we accept it as standard practice (and with an increased sample size). <\/default:p><default:p xmlns=\"http:\/\/www.w3.org\/1999\/xhtml\">I also would want baseline hormonal data before attributing all improvements solely to skin clearance.
